Effective December 1, 2017:

 

  • The debtor must file their chapter 13 plan using the local form Chapter 13 plan.  Note:  For plans filed before 12/1/2017 the debtor may file a modified plan using the prior chapter 13 form plan.

 

  • The form plan allows the debtor to include:

 

    • Motions for valuation;

    • Motions to avoid liens;

    • Request to surrender collateral; and

    • Request to assume executory contracts and/or unexpired leases
